  • Child's home address: This is the address where the child normally lives. If this is different from the applicant's address, please give the reason for this. If the applicant has moved house within the last six months, you will also need to provide evidence to confirm that your home address is permanent, rather than temporary. Parents/carers must provide documentary evidence as proof of permanent home address.

If you move house, or some other relevant change to your situation, please submit a new application with this new information, and provide new supporting documents. Please re-apply online or contact the school.

If this applies, you should indicate the reason and provide the appropriate documentation.

Proof of birth date: Parents/carers must provide documentary evidence showing proof of child's birth date.
