Prendergast Vale School is an all-through school for children aged 3 - 16 years. This form is for children applying to the Nursery.
Our nursery takes children from the term after they turn three years old.
All children are entitled to 15 hours of funded nursery education per week (term-time only) from the term after their third birthday. Some families may qualify for an extra 15 hours per week (30 hours total).
We offer both 15-hour part-time and 30-hour full-time places:
15 hours: • Mon–Tue, 8:45am–3:00pm + Wed morning, 8:45am–11:15am, or • Wed afternoon, 12:30pm–3:00pm + Thu–Fri, 8:45am–3:00pm
30 hours: • Mon–Thu, 8:45am–3:00pm + Fri, 8:45am–1:45pm*
To qualify for 30 hours’ funded childcare, you must meet government criteria and, in the term before your child joins, provide the school with your 11-digit eligibility code and National Insurance number for verification. Receiving 30 hours will not affect your eligibility for Universal Credit, tax credits, childcare vouchers, or Tax-Free Childcare. Learn more or apply at: www.gov.uk/30-hours-free-childcare

This application form must be completed by a parent/carer with parental responsibility to apply for a Nursery placement at Prendergast Vale School. We may require the applicant to provide formal evidence as proof of parental responsibility.
Child's home address: This is the address where the child normally lives. If this is different from the applicant's address, please give the reason for this. If the applicant has moved house within the last six months, you will also need to provide evidence to confirm that your home address is permanent, rather than temporary. Parents/carers must provide documentary evidence as proof of permanent home address.
If you move house, or some other relevant change to your situation, please submit a new application with this new information, and provide new supporting documents. Please re-apply online or contact the school.
Shared care arrangements: It is normally accepted that the child lives with the parent/carer who makes the application and is able to provide documentary evidence of the child’s main domiciliary address, that is, who the child lives with on school days. If this applies, you should indicate the reason and provide the appropriate documentation.
Proof of birth date: Parents/carers must provide documentary evidence showing proof of child's birth date.

The Applicant is Parent/Carer 1. Please include the name(s) of parent(s) or adults(s) with legal parental responsibility with whom the child lives permanently, including a parent with shared care. If you tick 'Other', provide evidence of legal parental responsibility.
Please ensure you complete a separate form for each child applying for a place in the Nursery. If you are also applying for place(s) in the main school (Reception-Year 11), please use our In-Year Application Form, available via the link on our In-Year Admissions webpage.
If you wish to make a claim for consideration under the criterion for exceptional social or medical need, please contact the school to ask for our Supplementary Information Form. A supplementary information form must accompany your application, with documentary evidence to support your claim. Please read our Nursery Admissions Policy carefully, before you submit your application.
Our school offers places in our Nursery in 15-hour sessions.
Parents may apply for:
- one 15-hour session (under the funded universal entitlement) which runs Monday-Wednesday; OR
- one 15-hour session (under the funded universal entitlement) which runs Wednesday-Friday OR
- two 15-hour sessions (under the funded extended entitlement) which run Monday-Friday
Note: If there are unfilled sessions in our nursery after offers have been made to all eligible children with a funded entitlement, these ‘spare’ 15-hour sessions can be purchased by parents without a 30-hour extended entitlement, in addition to their child’s 15-hour universal entitlement. Additional ‘bought’ sessions are guaranteed for one term only.
If you have selected a fully-funded 30-hour placement, you will need to provide your valid 30-hour eligibility code and your National Insurance number so that we can complete the required check.
Important: parents must apply for their 30-hour code during the term before their child is due to start the placement.
If you have selected a partially-funded 30-hour placement, please indicate your preferred 15-hour session under your child’s universal entitlement.
In the event a place is offered, this funded session under the universal entitlement will be guaranteed for the duration of your child’s enrolment in the nursery.
If your child is offered an additional non-funded 15-hour session, this ‘bought’ session will be guaranteed for one term only. All charges must be paid in full in advance of the start of the term.
If you have indicated your preference for a 30-hour placement (fully or partially funded), this will run as follows:
- Monday – Thursday, 8:45am – 3pm, and Friday, 8:45am – 1:45pm.
If you would like your child to attend until 3pm on a Friday, please indicate this below. A fee of £9.10 per week will be charged to cover the 1.25 hours of additional childcare above the 30-hour placement. All charges must be paid in full in advance of the start of the term.

I authorise the school and/or Local Authority to check the details I have supplied against any records held by the Local Authority, the child's current or previous childcare provision, as well as those of other external agencies.
Please make sure that you provide all documents (typically, a council tax statement/letter for the current year and your child's current passport). Failure to do so will delay consideration of your child’s application.
